About The Position

Comfort Hospice Care is seeking a Hospice RN Case Manager to deliver compassionate hospice care to patients in their homes and communities. The role supports patients and families through care decisions and provides comfort, dignity, and guidance during end-of-life care. Requirements

• Currently licensed as a Registered Nurse (RN) in the State of Nevada.
• Minimum of 2 years of nursing experience.
• Valid driver’s license and reliable, insured vehicle.
• Strong clinical skills and a passion for patient-centered care.

Nice To Haves

• Hospice or home health experience preferred.
• Homecare Homebase (HCHB) experience preferred, but not required.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans, revising them as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.
