About The Position
As a Hospice Field RN, you’ll provide one-on-one care that brings comfort, dignity, and peace to patients and families during life’s most important moments. The Hospice RN Case Manager provides compassionate, patient-centered care in the home or care facility setting while coordinating with an experienced hospice team.
Requirements
- Current RN License
- CPR Certification
- Valid Driver’s License & Auto Insurance
- Strong clinical, communication, and critical thinking skills
Nice To Haves
- Hospice RN experience preferred
- HomeCare HomeBase experience preferred
Responsibilities
- Performing comprehensive nursing assessments
- Developing and managing individualized care plans
- Providing symptom management and comfort-focused care
- Educating patients and families on disease progression and end-of-life care
- Collaborating with physicians, social workers, chaplains, aides, and the interdisciplinary team
- Supervising Hospice Aides and LPN/LVNs
- Monitoring patient condition and communicating changes promptly
- Completing accurate and timely documentation
- Supporting families emotionally through every stage of care
- Participating in on-call rotation as assigned
