About The Position
Inspire Hospice and Palliative Care is seeking a compassionate and dedicated RN Case Manager to join their team. This role involves providing comfort and support to patients and their families during end-of-life journeys. The ideal candidate will make patients feel at home, regardless of their care setting (home, assisted living, skilled nursing). Requirements
- Valid GA RN license
- Driver's license
- At least one year of experience in hospice or home health
Nice To Haves
- Hospice or home health experience
- CHPN, Certified Hospice and Palliative Nurse
Responsibilities
- Be a steady source of comfort and support to patients and families
- Help families understand disease progression, self-care, and nutrition
- Collaborate with the interdisciplinary hospice team to create and follow patient care plans
- Manage symptoms and advocate for patient comfort
- Maintain timely and accurate clinical documentation
