About The Position

Guthrie Hospice is seeking a full-time Hospice RN Case Manager to serve patients in the Towanda, PA area. This role involves coordinating and delivering high-quality, compassionate end-of-life care in collaboration with physicians, social workers, aides, chaplains, and the broader interdisciplinary team. The position offers professional autonomy and strong team support, allowing for management of the day while ensuring team support. The Hospice Nurse works under the general direction of the Director of Hospice and the attending physician in the provision of skilled nursing assessment, planning, and care to maximize patient and family comfort and health. The role involves identifying patient/family needs and providing supportive care in accordance with physician orders and the interdisciplinary plan of care. Direct supervision of Home Health Aides and/or LPNs providing care is also a responsibility. Travel and on-call duties are required.

Requirements

  • Registered Professional Nurse currently licensed in Pennsylvania.
  • Knowledge of nursing theory and practice at a level normally associated with an Associate's Degree.
  • One (1) year nursing experience in an acute care setting and/or previous Hospice experience or similar work with patients/families experiencing terminal illness is preferred.
  • Experience and skill in supervision desired.

Responsibilities

  • Coordinate and deliver high-quality, compassionate end-of-life care in collaboration with physicians, social workers, aides, chaplains, and the broader interdisciplinary team.
  • Act as the clinical lead for patients, managing their care.
  • Provide skilled nursing assessment, planning, and care to maximize the comfort and health of patients and families.
  • Identify patient/family needs and provide supportive care in accordance with the attending physician’s orders and the interdisciplinary plan of care.
  • Provide direct supervision of Home Health Aides and/or LPNs providing care.
  • Assume primary responsibility of a patient/family caseload, including accessing, planning, implementing, and evaluating the nursing process.
  • Initiate communication with attending physicians, other hospice staff members, and other agencies as needed to coordinate optimal care and resource utilization.
  • Maintain regular communication with Hospice director, Medical Director, and attending physician concerning caseload.
  • Obtain data on physical, psychological, social, and spiritual factors influencing patient/family status and incorporate into the plan of care.
  • Maintain up-to-date patient records, clearly stating problems, plans, actions, and goals, and reflecting changes as they occur.
  • Seek input from other team members regarding the patient plan of care for additional knowledge and support.
  • Coordinate physical care of the patient by teaching primary caregivers, volunteers, and employed caregivers (LPNs, hospice aides), or by providing direct care as appropriate.
  • Meet regularly with hospice nursing staff and attend interdisciplinary team meetings to review caseload issues, share professional support, and exchange feedback.
  • Supervise and document hospice aide plan of care every two weeks.
  • Supervise and document the practice of the LPN in the provision of patient care as delegated.
