Position Summary We are seeking a dedicated Hospice Registered Nurse (RN) Case Manager to provide skilled nursing care to hospice patients in accordance with physician orders, the State Nurse Practice Act, and agency policies. This role involves case management, direct patient care, collaboration with an interdisciplinary team, and supporting patients and families through end-of-life care.

Requirements

  • Current Registered Nurse (RN) license in Oklahoma (required)
  • Strong communication and documentation skills
  • Ability to adapt in a fast-paced, ever-changing environment
  • Valid driver’s license, reliable transportation, and active auto insurance
  • Strong attention to detail and organizational skills

Nice To Haves

  • Minimum of 2 years of hospice nursing experience preferred
  • Hospice & Palliative Nursing Certification (preferred)

Responsibilities

  • Provide direct nursing care to hospice patients in home and facility settings
  • Manage patient caseload and oversee the full hospice care process
  • Perform admissions, routine visits, evaluations, supervisory visits, and on-call duties
  • Collaborate with the interdisciplinary team to develop and implement care plans
  • Maintain accurate, timely, and compliant electronic documentation
  • Educate patients and families on disease progression, care goals, and symptom management
  • Participate in interdisciplinary team meetings and case conferences
  • Conduct supervisory visits for aides and LPNs as assigned
  • Support all levels of hospice care (routine, respite, continuous, inpatient, and imminent)
  • Respond to changes in patient condition and ensure appropriate follow-up
  • Participate in on-call rotation, including weekends and holidays
