About The Position

As an RN Case Manager with WesleyLife at Home Hospice, each day begins with purpose—the opportunity to bring comfort, dignity, and compassionate support to patients and families navigating one of life’s most meaningful journeys. During your visits as a Registered Nurse Case Manager you will: Travel to each patient to conduct thorough nursing assessments to monitor symptoms, comfort levels, and overall well‑being Manage pain and other symptoms using evidence‑based hospice practices Coordinate medications, treatments, and supplies to support patient needs Prepare and update individualized care plans in collaboration with the interdisciplinary team Provide education on what to expect throughout the hospice journey Teach family caregivers how to provide safe, comfort‑focused care Offer a listening ear and calm presence during moments of uncertainty WesleyLife Hospice provides services in-home and elsewhere; wherever we’re needed, we extend the personalized patient- and family-centered care we’re known for. Requirements

  • Valid, unrestricted Iowa driver’s license.
  • Current vehicle insurance.
  • Current license as a registered nurse in the state of Iowa.
  • Two years of full-time nursing experience required, with one year of hospice experience preferred.
