About The Position

Allay Hospice is seeking a passionate, dedicated Hospice RN to join our team! Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. We offer lower patient-to-nurse ratios for meaningful, unrushed hospice care, strong interdisciplinary team support, leadership that understands hospice nursing, and a dedicated internal after-hours triage team. We utilize Homecare Homebase (HCHB) EMR for streamlined documentation. The office is located at 735 Cornerstone Crossing, Waterford, WI 53185, and travel throughout the territory is required. The schedule is Monday-Friday 8 AM-4:30 PM.

Requirements

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

Nice To Haves

• 3+ years preferred
• Experience in hospice or a similar setting preferred

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
