About The Position

The Hospice RN, Case Manager plans, organizes and directs hospice care services and is experienced in nursing, with emphasis on community health education/experience. The RN, Case Manager builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. This is a full-time position and will supporting the Covenant Living of Holmstad location (Batavia) and the surrounding locations.

Requirements

  • Associate’s Degree, Nursing
  • RN License and CPR, if required by state
  • 1+ Years of hospice care experience preferred
  • Ability to communicate effectively with residents and healthcare professionals.
  • Knowledge of cultural, social, and economic factors that influence patient care and healthcare access.
  • Ability to report and document financial information as needed.
  • Knowledge of privacy regulations and maintaining patient confidentiality.

Responsibilities

  • Completes an initial assessment of patient and family to determine hospice care needs.
  • Provides a complete physical assessment and history of current and previous illness(es).
  • Regularly re-evaluates patient nursing needs.
  • Initiates and oversees the plan of care and makes necessary revisions as patient status and needs change. Communicates updates as necessary.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • Administers medications and treatments as prescribed by the physician.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan of care.
  • Acts as member of Interdisciplinary Team and participates in IDT meetings and case conferences.
