About The Position
Sentara Health is seeking to hire a qualified individual to join our team as a Registered Nurse Case Manager. The Direct Care Registered Nurse - Home Health and Hospice Provides high-quality nursing care to patients in their homes or hospice settings, ensuring comfort, dignity, and emotional support during end-of-life care.
Requirements
- Associate's Degree in Nursing (Required)
- Valid RN License in state of practice (VA) (Required)
- BLS (Required within 90 days of hire)
- 1 year of nursing experience (Required)
Nice To Haves
- Previous Home Care/Hospice experience (Preferred)
- Prior case management experience (Preferred)
