About The Position

Puget Sound Home Health and Hospice is seeking an experienced and caring full-time Hospice Registered Nurse Case Manager (RN) in Pierce County, Washington! This role involves completing comprehensive in-home patient assessments, managing patient comfort, symptom control, and pain relief, and developing and updating individualized plans of care under physician orders. The RN will deliver skilled, compassionate nursing care in the home setting, document care accurately and promptly, participate in quality improvement initiatives, and collaborate with an interdisciplinary team for holistic patient support.

Requirements

  • Current RN license in WA (or compact state)
  • BLS certification from AHA
  • Valid driver’s license, auto insurance, and reliable transportation
  • Strong critical thinking and independent problem-solving skills

Nice To Haves

  • Hospice agency experience preferred but not required

Responsibilities

  • Complete comprehensive in-home patient assessments
  • Manage patient comfort, symptom control, and pain relief
  • Develop and update individualized plans of care under physician orders
  • Deliver skilled, compassionate nursing care in the home setting
  • Document care accurately and promptly per clinical and legal standards
  • Participate in quality improvement initiatives (QAPI)
  • Collaborate with an interdisciplinary team to ensure holistic patient support
