About The Position

Hospice of Marion County, a part of Empath Health is seeking a caring and motivated Admissions RN for admissions in homes, facilities and hospitals in Marion County. Since 1983, Hospice of Marion County has been honored to serve families across Ocala and Marion County with compassionate, expert end-of-life care. As part of Empath Health, we help neighbors with advanced illness live as comfortably and meaningfully as possible, while supporting their families and caregivers with respect, guidance, and dignity.

Requirements

• Associate’s degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
• Valid Florida license as a Registered Nurse
• Valid Florida State Driver’s License
• For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.

Nice To Haves

• Hospice experience strongly preferred

Responsibilities

• The Admissions RN is a first responder who assesses eligibility for Empath Health Network of Care
• Advocating for optimal access to the right care at the right time in any care setting.
• The Admissions RN completes the admission process based on care program specifics, guidelines, and patient-specific needs.
• Coordinate and assist with the Professional Liaison to deliver educational, in-service and outreach tools in the hospital.
• Works directly with community referral sources to educate regarding Empath Health programs and services.
• Complies with RN On Call scheduling needs as well as vendor credentialing process.
