About The Position

Named by the Boston Globe as one of the Top Places to Work in Massachusetts for the last three years, our community-based, mission-driven hospice and palliative care organization seeks a Hospice Residence LPN to join its expanding hospice team in our residence. Our Hospice Residence is located in Wayland and was reopened in the Fall of 2022. This is an exciting opportunity for LPNs interested in providing end-of-life care at a new 10-bed hospice facility. This position is eligible for a $5000 sign-on Bonus.

Requirements

  • Licensed Practical Nurse (MA license)
  • Ability to be a team player and work harmoniously with patients, families, co-workers, and management.
  • Providing care consistent with written instructions.

Nice To Haves

  • A minimum of two years prior experience in community health, long term care facility, oncology, or a related field preferred.

Responsibilities

  • Provides direct patient care, including ongoing assessment, pain management, and symptom control in line with the plan of care established for the patient.
  • Notifies the Nursing Care Coordinator of any significant changes in the patient’s condition or any changes that necessitate physician notification.
  • Documents all care provided, including clinical observations and interventions.
  • Provides assessments of the impact of the terminal diagnosis on the patient’s physical, functional and environmental needs and activities of daily living.
  • Provides on-going evaluation of patient/family response to care and informing the Nursing Care Coordinator and other personnel of changes in the patient’s needs and outcomes of interventions.
  • Prepares documentation that demonstrates adherence to the patient’s individualized plan of care and progress towards established goals.
  • Assesses for patients’ safety.
  • Provides personal care including assistance with daily hygiene, ambulation, exercise, toileting, nutritional support, dressing changes and other care as designated by the Nursing Care Coordinator.
  • Assists with medications that are ordinarily self-administered.
  • Assists patients with household activities.
