About The Position
Elara Caring is seeking a compassionate Registered Nurse to provide nursing care to patients in a home care setting. The nurse assesses patients’ needs, provides care in accordance with the plan of care and physician orders, monitors conditions, and supports safe, continuous, quality care. The position involves educating patients and families, protecting privacy under HIPAA, and responding appropriately to changes in patient conditions. The role requires travel between patient homes and assigned work locations.
Requirements
- Current state license as a Registered Nurse.
- Graduate of an accredited certificate, diploma, associate, or baccalaureate school of nursing.
- At least 1 year of experience as a nurse in a clinical care setting.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Valid driver’s license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Responsibilities
- Assess patients to determine their care needs.
- Complete initial and ongoing patient assessments.
- Protect patient and staff privacy in accordance with HIPAA.
- Provide nursing care according to the plan of care and physician orders.
- Monitor patient conditions and report changes to the care team.
- Educate patients and families on care and safety.
- Take appropriate action when patient conditions change.
- Help ensure quality, safe, and continuous patient care.
