Hiring Hospice Registered Nurse in Lexington, OH with $2200 Weekly Gross
About The Position
Sage Group HealthCare Staffing is seeking a Hospice RN for multiple travel and contract positions in Lexington, OH. We accept both local and traveler candidates. This role involves providing and coordinating care for assigned hospice patients in their homes, developing and implementing nursing care plans, and supporting patient comfort and dignity. The nurse will empower patients and families by educating them on pain management and providing home health nursing care within the scope of practice. Key responsibilities include assessing patient status, documenting visits in a timely manner, and communicating with physicians and the office promptly. The territories to cover include Mansfield, OH, and surrounding cities.
Requirements
- Registered Nurse: 2 years - Required
- Recent/Current Hospice Nursing experience in a home setting: 1 year within the last 3 years – Required
- Home Care Home Base (HCHB) charting experience – Required
- Timely communication and documentation - Required
- Must have valid Driver’s License and Personal Auto Insurance
- Must be Covid Vaccinated or provide medical or religious exemption
- ASN or an equivalent – Required
- RN License for State of Ohio or Compact - Required
- BLS Certification from AHA or American Red Cross - Required
Nice To Haves
- BSN or an equivalent – Highly preferred
Responsibilities
- Provide and coordinate care for assigned hospice patients in comfort of their homes.
- Develop and implement nursing care plans for all type of assigned visits.
- Support patient comfort and dignity.
- Empower patients and families to reach healthcare goals by educating them on pain management.
- Provide Home health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status.
- Communicate with the office promptly.
- Document and complete charting for all patient visits in a timely manner in accordance with client policies and procedures.
