About The Position

Health Link Home Health & Hospice is seeking an experienced, compassionate Per Diem Hospice Registered Nurse (RN) to join their growing East Bay hospice program. The role involves providing skilled nursing care to patients with life-limiting illnesses, focusing on comfort and family support. The position requires strong assessment skills, compassionate communication, clinical judgment, and the ability to work independently within an interdisciplinary team. The company emphasizes allowing nurses to focus on patient care by handling administrative tasks like scheduling, referrals, and authorizations.

Requirements

  • Current California Registered Nurse (RN) license in good standing.
  • Minimum one (1) year of RN experience required.
  • Excellent assessment and critical thinking skills.
  • Compassionate, dependable, and patient-centered.
  • Strong communication and organizational abilities.
  • Ability to work independently while collaborating with an interdisciplinary team.
  • Current California driver's license.
  • Reliable transportation with proof of auto insurance.
  • Current CPR/BLS certification.

Nice To Haves

  • Previous Hospice or Home Health experience strongly preferred.
  • Hospice Nursing experience.
  • Home Health Nursing experience.

Responsibilities

  • Perform comprehensive hospice nursing assessments.
  • Develop, implement, and evaluate individualized plans of care.
  • Provide skilled nursing visits in patients' homes, assisted living facilities, board and care homes, and skilled nursing facilities.
  • Assess and manage pain and symptoms using evidence-based hospice practices.
  • Administer medications, treatments, and interventions as ordered.
  • Educate patients and caregivers regarding disease progression, medications, symptom management, and end-of-life care.
  • Coordinate care with physicians, social workers, chaplains, home health aides, and the interdisciplinary team.
  • Communicate changes in patient condition promptly to the Hospice Director and attending physician.
  • Complete accurate and timely electronic documentation using WellSky (Kinnser).
  • Participate in Interdisciplinary Group (IDG) meetings.
  • Maintain compliance with Medicare Conditions of Participation, California regulations, and agency policies.
  • Participate in Quality Assurance and Performance Improvement (QAPI) activities as assigned.
