About The Position

This full-time Hospice Registered Nurse position focuses on case management within the hospice setting. The role involves prioritizing patient needs, evaluating and revising care plans, and demonstrating expertise in pharmacology, medication management, and end-of-life symptom management. The nurse will also be involved in reconciling medications, maintaining current medication orders, and ensuring interdisciplinary communication. Additionally, the position contributes to agency growth, assists with new employee orientation, and encourages cross-training and mutual teaching among staff.

Requirements

  • Graduate of an accredited Nursing Program.
  • Current state licensure as a Registered Nurse.
  • BLS certification.

Nice To Haves

  • Bachelor’s degree in nursing.
  • All ASN must complete their BSN studies within five years of their start date.
  • 1+ year of experience in Nursing or Hospice.

Responsibilities

  • Performs patient assessments, including appropriateness for hospice care, learning needs, and progress toward goals.
  • Develops and revises the plan of care for patients and families.
  • Ensures appropriate utilization of services and maintains visit productivity standards.
  • Demonstrates knowledge of insurance reimbursement guidelines.
  • Completes discharge assessments.
  • Performs laptop transfer of data twice daily.
  • Completes admission assessment and transfers within 24 hours.
  • Completes re-visit documentation and transfers.
  • Uses age-appropriate teaching methods for patients and families.
  • Evaluates teaching plans and revises as necessary.
  • Documents patient education tools used.
  • Assumes responsibility for Case Management, including developing, implementing, and evaluating the plan of care based on evidence-based knowledge.
  • Prioritizes care needs.
  • Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines.
  • Shows evidence of interdisciplinary communication.
  • Performs HHA supervisions per standards.
  • Follows scheduling process.
  • Communicates progress toward goals with the Interdisciplinary Team (IDT).
  • Attends and participates in IDT meetings every 2 weeks.
  • Maintains a safe environment for patients and co-workers.
  • Follows infection control measures and reports infections.
  • Demonstrates knowledge of pharmacology and safe medication management.
  • Routinely reconciles medications.
  • Maintains current medication orders.
  • Attends monthly staff meetings.
  • Participates in PI meetings and activities/committees as requested.
  • Accepts other assignments as requested.
  • Documents case management activities.
  • Assists with growing agency business.
  • Provides orientation to new employees.
  • Cross trains to learn new skills and demonstrates a willingness to teach each other.
