About The Position

The Hospice Registered Nurse provides individualized nursing care to maximize comfort and quality of life for hospice patients. The RN works as part of an interdisciplinary care team, building relationships with patients, families, physicians, and teammates while practicing autonomously with organizational support. The role involves comprehensive assessment of patients’ physical, emotional, social, and spiritual needs; care planning and coordination; patient and caregiver education; and accurate documentation. The schedule is PRN days as needed, covering visits to patients in ZIP codes 33015, 33016, 33018, 33055, 33053, and 33162. Participation in on-call or admission rotations may be needed.

Requirements

Registered Nurse (RN) license in the state of practice.
At least one year of nursing experience.
Reliable transportation, a valid driver’s license, and appropriate auto insurance.

Partner with patients, families, physicians, and the interdisciplinary group to create and carry out care plans.
Perform comprehensive physical, emotional, social, and spiritual assessments to guide care decisions.
Educate and support patients and caregivers.
Coordinate care across disciplines to ensure continuity and dignity.
Document care clearly and accurately to reflect patient needs and progress.
Participate in on-call or admission rotations as needed.
Engage in ongoing learning and professional development.
