About The Position

CareFor is seeking a compassionate and dedicated Registered Nurse to join their hospice care team. This role involves serving as a vital guide for patients and families navigating the end-of-life journey. The Hospice RN Care Manager will build trusting relationships in patients' homes, providing comprehensive and compassionate care to ensure comfort and quality of life during their final days. Responsibilities include assessing, planning, and implementing individualized care plans for multiple patients, and providing essential support and education to families. CareFor emphasizes a supportive management team, professional growth, and a culture of feedback, aiming to provide high-quality, personalized hospice care in Central Texas.

Requirements

  • Current RN License: Active in Texas and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact.
  • 1 year of nursing experience required.
  • A valid driver’s license, insurance, and reliable transportation to and around the South Austin area (including South Austin, Dripping Springs, Kyle, Buda, and surrounding areas).
  • Maturity and the ability to deal effectively with the emotional and physical demands of the job.
  • Ability to carry out directions, read, and write with precision.

Nice To Haves

  • 2 years of nursing experience preferred.
  • Experience in hospice or palliative care is highly valued.

Responsibilities

  • Conduct initial and ongoing comprehensive assessments of the terminal disease’s impact on patients’ physical, functional, psychosocial, and environmental needs.
  • Implement and document personalized care plans to ensure quality and continuity, making necessary revisions based on patient needs and outcomes.
  • Act as a trusted consultant and educator for patients and families, explaining disease processes, teaching self-care techniques, guiding end-of-life care, and addressing dietary needs.
  • Determine the scope and frequency of services needed based on the patient’s condition and coordinate with the interdisciplinary team.
  • Initiate appropriate preventive and rehabilitative nursing procedures, applying strict infection control practices.
  • Prepare clinical and progress notes that reflect progress toward established goals and ensure compliance with third-party payor requirements.
  • Maintain effective, empathetic communication with patients, families, team members, and other healthcare professionals, keeping physicians informed of changes in patient needs.
  • Provide on-call services during business hours to ensure patient and family support during emergencies.
  • Participate in hospice-sponsored in-service programs, maintain professional growth, and contribute to the hospice’s Quality Assessment and Performance Improvement (QAPI) program.
  • Provide training for other staff as needed.
