Eden Health•Longview, WA

About The Position

The Hospice Registered Nurse provides specialized nursing services according to each patient’s plan of care. Caseloads generally range from approximately 12 to 18 patients and vary based on patient and family needs, geography, and whether care is provided at home or in a facility. The RN demonstrates clinical competency in pain management, symptom control, infusion therapy and device care, wound care, and aseptic technique, and uses critical thinking and clinical judgment when caring for patients and families facing terminal illness. The RN addresses the physical, emotional, psychosocial, and spiritual needs of patients and their families or caregivers. As the patient’s case manager, the RN coordinates and evaluates the plan of care from admission through discharge, maintains continuity with other health professionals, communicates with the attending physician and Hospice Medical Director about changes in condition, and coordinates with the interdisciplinary team on bereavement. The RN also educates patients and families about their rights and responsibilities, makes referrals as needed, attends interdisciplinary meetings, and initiates care conferences for complex patients when needed.
