About The Position

Grace Healthcare Services is seeking a Hospice Registered Nurse to join its Interdisciplinary Team in Sussex County, New Jersey. The RN assesses and addresses the comprehensive needs of patients and their families, initiates appropriate interventions upon admission, and helps ensure a continuous, responsive plan of care throughout the patient’s journey. The role focuses on compassionate care that supports the comfort and quality of life of terminally ill patients and treats patients and families with dignity and respect. The schedule is Monday through Friday, 8:30 a.m. to 5:00 p.m. The position requires daily travel to assigned patient locations and facilities.

Requirements

Valid New Jersey or Multi-State Registered Nurse (RN) license.
At least two years of nursing experience in acute-care settings such as medical, surgical, or oncology units.
Strong clinical practice.
Excellent communication abilities.
Customer-service focus.
Ability to work independently in a fast-paced environment.

Responsibilities

Deliver skilled nursing care according to individualized care plans, supporting comfort and quality of life for terminally ill patients.
Collaborate with physicians, social workers, chaplains, nursing assistants, and other stakeholders to develop and adjust patient care plans.
Educate and support patients’ families and primary caregivers, including guidance on care techniques and emotional support.
Liaise among patients, families, and other healthcare providers to support clear communication and advocate for patients’ wishes and needs.
Travel daily to assigned patient locations and facilities.
Assess patients’ and families’ comprehensive needs, initiate appropriate interventions upon admission, and support a continuous, responsive plan of care.
