About The Position

This is a full-time Registered Nurse (RN) opening in the hospice division, serving patients in their own homes or facilities/nursing homes for Home Health & Hospice Care. The primary function involves admissions, with additional visits as needed. Nurses have the ability to work autonomously once trained, with exceptional team support from a highly skilled Manager and Team Lead specializing in hospice/end-of-life care. Hospice care is presented as a specialty line of nursing with growth possibilities. The role is 40 hours per week, Monday to Friday, with weekend and holiday rotation. Requirements

  • Graduate of an accredited school of nursing.
  • Associate degree in nursing.
  • Registered as RN with New Hampshire’s Board of Nursing.
  • A minimum of one year recent acute care experience.
  • Ability to exercise initiative and independent judgment.
  • Once fully trained minimum requirements is one weekend a month.
  • 1 winter and 1 summer agency holidays required.
  • CPR Certification.
  • Satisfactory criminal record.
  • Satisfactory motor vehicle record.

Nice To Haves

  • EPIC experience preferred, willing to train!
  • Bachelor degree in nursing.
  • Community Health/Education experience.
  • Certification in Hospice and Palliative Care, Oncology or end-of-life experience is preferred, but willing to train the right candidate.
  • EMR software proficiency.
  • Registered as a RN with New Hampshire’s Board of Nursing.

Responsibilities

  • Completes an initial home health assessment of patient and family to determine hospice care needs.
  • Provides a complete physical assessment and history of current and previous illness(es).
  • Utilizes the nursing process in planning, providing and evaluating skilled needs.
  • Completes a comprehensive patient assessment.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
  • Includes the patient and family on the planning and revising process.
