About The Position

This is a full-time Hospice Registered Nurse position at AdvisaCare, focusing on providing compassionate, one-on-one care in patients' homes. The role emphasizes honoring each moment, supporting patients and families with dignity, and allowing nurses to spend meaningful time with those in their care. It offers a flexible schedule, working 30-40 hours per week, and includes rotation of the on-call schedule. The Hospice RN will provide emotional support, aiming to help patients live their final days with dignity and comfort. Responsibilities include conducting supervisory visits for clinical observation of assigned team members and participating in interdisciplinary group meetings for the development, review, and revision of the patient/family plan of care.

Requirements

  • Current Michigan License in good standing
  • Minimum 1 year hospice experience
  • Empathetic approach to patients and caregivers
  • Ability to adapt to patient’s status and needs on a full time on call basis

Responsibilities

  • Provide one-on-one care in the comfort of home
  • Support patients and families with dignity and heart

  • Conduct appropriate supervisory visits for clinical observation of performance of assigned team members
  • Participate and provide feedback to the team for regular interdisciplinary group meetings
  • Participate in the development, review and revision of the patient/family plan of care
  • Conduct initial and on-going assessment of patient’s physical, functional, psychosocial and environmental needs
  • Implement the individualized plan of care and revisions as needed
  • Consult with and educate the patient/family on disease process, self-care techniques, end-of-life care, and dealing with issues of ethical concern
  • Supervise paraprofessionals providing services to patients
  • Prepare clinical and progress notes
  • Participate in IDG conferences
  • Participate in Hospice performance improvement program
  • Maintain high level of customer satisfaction with clients, families, and caregivers
  • Communicate information using current process and technology within the organization
  • Participate in field visits daily
  • Participate in weekly office meetings as indicated
