About The Position

AdvisaCare is expanding its Hospice services by adding a new location in Livonia, with the Troy agency serving as the parent office. The company is actively seeking compassionate, dynamic, energetic, organized, and efficient professionals with Hospice experience to join their growing Hospice Family. This full-time Hospice RN role is crucial for helping to establish and run the new service. The position requires a registered nurse who values Hospice care and is dedicated to providing emotional support to patients, with the ultimate goal of helping them live their final days with dignity and comfort.

Requirements

Current Michigan License in good standing
Minimum 1 year hospice experience
Empathetic approach to patients and caregivers
Participate in field visits daily, weekly office meetings as indicated
Ability to adapt to patient’s status and needs on a full time on call basis
Registered nurse who sees the value of Hospice care
Position requires On-Call responsibilities (Current On Call Hours - 5 pm - 8 am Available!)

Responsibilities

Conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members
Participate and provide feedback to the team for regular interdisciplinary group meetings
Participate in the development, review and revision of the patient/family plan of care
Initial and on-going assessment of patient’s physical, functional, psychosocial and environmental needs
Implement the individualized plan of care and revisions as needed
Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance
Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern
Supervision of paraprofessionals providing services to patient
Preparing clinical and progress notes
Participating in IDG conferences
Participate in Hospice performance improvement program
Maintain high level of customer satisfaction with our clients, families and caregivers
Communication of information using current process and technology within organization
