About The Position
Sage Group HealthCare Staffing is seeking to hire a Hospice RN for a contract position in Tukwila, WA (Seattle region). This is a full-time, temporary contract role for 13 weeks, with a guaranteed 40 hours per week. The position involves providing and coordinating care, case management, and support for assigned hospice patients in their homes. Responsibilities include admissions, developing care plans, performing various types of visits, supporting patient comfort and pain management, educating patients and families, and providing bereavement support. The role requires adherence to state Nurse Practice Act regulations and facility policies. The nurse will be responsible for assessing patients, documenting changes, transcribing physician orders, and completing charting within 24 hours. This is a float position covering areas in and around Seattle and South King County.
Requirements
- Registered Nurse: 1 year - Required
- Recent/Current Hospice nursing experience in a Home Health setting: 1 year within the last 3 years – Required
- Home Care Home Base (HCHB) charting - Required
- Must have valid Driver’s License and Personal Auto Insurance
- ASN or an equivalent – Required
- RN License for State of the state applying or Compact if acceptable - Required
- BLS Certification from AHA or American Red Cross - Required
Nice To Haves
- BSN or an equivalent – Highly preferred
Responsibilities
- Provide and coordinate care and case management for assigned hospice patients in comfort of their homes including admissions
- Develop and implement nursing care plans for visits as assigned.
- Performs Case Management and all types of visits for the assigned patient case load
- Support patient comfort, pain management and ability to stay at home - enabling them to spend quality time with their loved ones.
- Empower patients and families by educating them on disease management and pain management.
- Provide bereavement support.
- Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status.
- Document and complete charting for all patient visits within 24 hours.
- Provide bereavement support to patient’s families as needed.
