About The Position

Requirements
- Current RN license specific to the state(s) where the employee is assigned to work.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and liability insurance.

- At least 1 year of nursing experience.
Responsibilities
- Performs initial nursing assessments for new admissions.
- Completes, coordinates, and revises the total plan of care, maintaining continuity of patient care through collaboration with appropriate staff.
- Regularly communicates patient progress to the clinical manager, physician, and care team.
- Develops, prepares, and maintains individualized patient care progress records accurately and promptly according to care center policies.
- Records changes and outcomes related to pain and symptom management as appropriate.
- Educates families about the patient’s condition, needs, and care.
- Makes referrals to other hospice care disciplines as needed, or documents the rationale for not doing so.
- Performs on-call responsibilities and provides on-call services to patients and families as assigned.
- Performs other duties as assigned.
