Requirements

Current RN licensure in the state of Kentucky and/or Indiana as required by designated position, or the ability to obtain required license upon hire.
Minimum of 1 year of nursing experience.
Knowledge of palliative care required.
Must have a valid driver’s license in accordance to policy.

Nice To Haves

BSN preferred.
CPHN encouraged.
Previous home health or hospice experience preferred.
Use of technology-based tools, including electronic medical record, strongly preferred.

Responsibilities

Responds to on-call services and upholds the nursing service standards of care throughout the organization.
May make visits to home or facility patients, including scheduled and unplanned visits.
Follows up on new admissions as needed.
Conducts admission visits for medically eligible patients into Hosparus care.
Conducts facility to home admissions as needed.
Demonstrates excellence in nursing practice for assignments.
Supports the organization’s needs in providing nursing care and services to all Hosparus Health patients.
Responsible for compliance with regulatory and accreditation standards.
