About The Position

Adoration Home Health and Hospice is seeking a passionate, dedicated Home Health RN to join our team in Clanton, AL. Our Home Health RNs provide expert, patient-centered care. This role offers a supportive, fulfilling environment where your skills and empathy can shine, with a $5K Sign On bonus. The office is located in Clanton, AL, and the coverage area includes Shelby and Chilton Counties. This is a Full-Time position, Monday to Friday, 8 am to 5:30 pm.

Requirements

• Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the applicable state
• One year nursing practice in a patient care setting
• Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy
• Current CPR certification

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create home health care plans that align with MD orders and the patient's goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with an interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by the local branch
