About The Position
JourneyCare Hospice is seeking a dedicated Hospice Registered Nurse (RN) to provide compassionate, patient-centered care in the Barrington, IL. area. This is an excellent opportunity for nurses with hospice, home health, or acute care experience looking to make a meaningful impact. Requirements
- Active RN license in Illinois
- Graduate of an accredited nursing program
- Current CPR certification
- Minimum 1 year of hospice experience preferred (home health or med-surg considered)
- Strong assessment and critical thinking skills
- Ability to work independently in the field
- Excellent communication and patient care skills
Responsibilities
- Perform hospice admissions and pre-admission assessments
- Educate patients and families on hospice care and services
- Develop and implement individualized care plans
- Provide hands-on nursing care focused on comfort and quality of life
- Collaborate with interdisciplinary care teams
- Ensure compliance with all clinical and quality standards
