Requirements

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State License as a Registered Nurse
  • Minimum one (2) year experience as a nurse in a hospice or home health environment
  • Nursing skills as defined by accepted nursing standards
  • Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
  • Possess excellent observation, analytical, verbal, and written communication skills
  • Demonstrates the ability to assess and respond to the needs of the organization
  • Strong leadership skills to be able to train clinicians
  • Must be able to communicate professionally with all levels of the organization
  • Reliable transportation and valid automobile insurance coverage

Responsibilities

  • Administers skilled nursing care to terminally ill patients and families in home or facility settings.
  • Performs admission, routine, respite, or continuous care visits and assesses patients within regulatory requirements.
  • Responsible for overall coordination of care for assigned patient caseload.
  • Makes regular visits to patients, communicates with the Interdisciplinary Team (IDG) and Medical Director.
  • Identifies and addresses patient, family, and caregiver needs in accordance with physician orders and the IDG plan of care.
  • Responsible for the training of clinicians, including new hire orientation and remedial training.
  • May perform facility education when requested.
  • Makes follow-up visits according to patient, family, and facility needs.
  • Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.
  • Possesses basic assessment skills specific to Hospice care, including identification of home safety risks, appropriateness of home environment, availability of support systems, and community resources.
  • Initiates, supervises, and documents the observance of Home Health Aide and LPN care delivery per organization policy.
  • Assumes responsibility for the clinical training of students and remedial training of tenured staff.
  • Maintains regular communication with Education Departments and student supervisors.
  • Ensures students meet Optimal Care expectations for productivity, time to synch, timely documentation completion, length of visits, and other defined metrics.
