About The Position

The Hospice Registered Nurse Case Manager delivers skilled nursing care in patients’ homes. The role includes creating individualized care plans, administering medications, and educating patients and families. The position provides meaningful one-to-one patient care and is based in the Muskegon, Michigan coverage area.

Requirements

• Active RN license in Michigan.
• At least 1 year of clinical nursing experience; hospice experience is preferred.
• Strong communication, assessment, and organizational skills.
• Compassionate, dependable, and mission-minded.
• Valid driver’s license and reliable transportation.
