About The Position

Suncrest Hospice is seeking a Registered Nurse (RN) to care for patients on a PRN (as needed) basis. The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, and visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.

Requirements

• Current Registered Nurse (RN) licensure (in good standing) in the state of Ohio.
• Ability to work well within a team environment.
• Ability to work independently on a day-to-day basis.
• Understanding of hospice rules and regulations.
• Time management skills.
• Ability to learn an electronic medical records system.
• Willingness to drive with reliable transportation, a valid driver’s license, and auto insurance.

Nice To Haves

• Experience in hospice care.
• Expertise in psych-social assessments, interventions, and knowledge of community sources.

Responsibilities

• Providing care and services for a group of patients.
• Visiting patients where they reside (private homes, Skilled Nursing Facilities, assisted living facilities).
• Providing expertise regarding psych-social assessments, interventions, and knowledge of community sources as a member of the interdisciplinary team.
• Collaborating daily with nurses, social workers, chaplains, volunteers, and aide leadership.
• Working closely with patients on a 1-1 level.
• Managing visits, documentation, and meetings.
• Responding to urgent needs by prioritizing and triaging multiple concerns.
• Driving across the service area.
