About The Position

Elara Caring is seeking a compassionate Registered Nurse Case Manager to provide hospice care and support patients and families. The role assesses patients’ physical, psychological, social, spiritual, and environmental needs; monitors changes in condition; and supports implementation of the interdisciplinary Plan of Care. The nurse also educates patients, families, caregivers, and staff in care settings on hospice philosophy, pain management, and symptom management, and provides support at the time of death and bereavement assessments.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

• Experience in a hospice or home health environment.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Educate staff in nursing homes, hospitals, assisted living facilities, and other care settings on hospice philosophy, pain management, and symptom management.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
