Hospice - Registered Nurse - Case Manager - Milwaukee
About The Position
This is a meaningful work opportunity for a Hospice RN Case Manager in Milwaukee County. The role offers a competitive pay of up to $45/hr and a $10,000 sign-on bonus. The position is designed for a work-life balance with a 15-18 patient caseload average, 5-6 visits per day, and a Monday-Friday schedule totaling 40 hours. There is no routine on-call or weekend work, with limitations only for vacation and sick coverage. A dedicated after-hours on-call team and a company vehicle are provided for travel within Milwaukee County. The role is within a collaborative and supportive Interdisciplinary Care Team (IDG). Hospice experience is not required. This position is ideal for an experienced RN seeking an alternative to hospital burnout while continuing their nursing career.
Requirements
- Registered Nurse (RN) license.
- Experience as an RN.
- Willingness to transition from hospital nursing.
- Ability to work within Milwaukee County.
Nice To Haves
- Hospice experience is not required, but preferred.
- Experience with home health or hospice agencies.
- Experience in senior living communities.
