Hospice Registered Nurse Case Manager
About The Position
St. Mary's Home care & Hospice is seeking a full time Hospice RN Case Manager. This position is primarily Monday-Friday with no on-call or weekend requirements. The RN Case Manager will be responsible for case managing a team of hospice patients, collaborating with an interdisciplinary team, attending weekly Interdisciplinary meetings, and interacting with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms. Requirements
- Graduate of an approved nursing education program
- Licensure as a Registered Nurse in the State of Georgia
- 2 years experience as a professional nurse in acute care, home care or ER experience required
Nice To Haves
- Home care experience preferred but not required
- EPIC experience preferred
