About The Position

A Hospice & Palliative Care Nurse Practitioner is a member of the Hospice & Palliative Medical Team and delivers care to patients in a variety of settings. The Nurse Practitioner conducts Face to Face encounters with palliative patients and makes recommendations to the Interdisciplinary Team (IDT) regarding patient needs and symptom management.

Requirements

• Current unrestricted license to practice as a Nurse Practitioner.
• Graduate of an approved Nurse Practitioner program.
• Current CPR certification.
• Minimum of 3 years of palliative care and/or hospice experience.
• Excellent observation, verbal, and written communication skills.
• Valid Driver's License
• Reliable transportation and auto insurance

Responsibilities

• Conducts face to face visits for palliative patients in all settings.
• This may include physical examination, interpretation of health data, prescribing medications in accordance with state law, ordering routine and diagnostic studies and collaborating with physicians as appropriate.
• Consults with medical colleagues as needed, communicates about patient care provided, and provides reports as required.
• Assists patients and families in determining goals of medical care.
