About The Position

The On-Call LVN/LPN will be responsible for providing care and services for a group of hospice patients, visiting patients where they reside, which could include private homes, assisted living facilities and hospitals. This position will be after hours on-call working seven days/nights on and seven nights/days off 5PM-8:30AM.

Requirements

• Current LVN/LPN licensure (in good standing) in the state(s) of practice.
• One year of experience under license.
• Works well within a team environment.
• Ability to learn an electronic medical records system.
• Willingness to drive with reliable transportation, valid driver’s license, and auto insurance.

Nice To Haves

• On-call experience preferred.
• Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
• Time management skills.

Responsibilities

• Providing care and services for a group of hospice patients.
• Visiting patients where they reside, which could include private homes, assisted living facilities and hospitals.
• After hours on-call working seven days/nights on and seven nights/days off 5PM-8:30AM.
• Collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Working closely with patients on a 1-1 level.
• Managing visits, documentation, and meetings.
• Responding to urgent needs by prioritizing and triaging multiple concerns.
• Driving with reliable transportation to travel across the service area.
