Requirements

  • Minimum 1 year experience required.

Responsibilities

  • Delivery of patient care including personal care (ADL) such as bathing, body mechanics, nutrition, elimination, and safety procedures.
  • Performing special procedures including vital signs, height and weight, application of heat/cold, prevention and care for decubitus ulcers, surgical skin preps and scrubs, clean dressing changes, application of ace bandages, TEDs and binders, postmortem care, and assisting with cough/deep breathing.
  • Performing housekeeping services necessary for the health and safety of the patient/family/caregiver.
  • Participating in interdisciplinary team conferences (IDT) and special case conferences.
  • If functioning as a NA 1+ 4, providing personal care (ADL) plus sterile dressing for wounds over 48 hours old; wound irrigation; ostomy care or ostomy irrigation; insertion of urinary catheter (replacement only); and oxygen therapy.
  • If functioning as a NAII, providing personal care (ADL) plus breaking-up and removal of fecal impaction; IV Fluid- Assisted Activities; nutritional activities (oral/nasogastric infusions, gastrostomy feedings); suctioning (Oropharyngeal, nasopharyngeal); tracheostomy care; insertion of urinary catheter (replacement only); sterile dressing for wounds over 48 hours old; wound irrigation; ostomy care or ostomy irrigation; and oxygen therapy.
  • Attending and participating in interdisciplinary team conferences [IDT] and special care conferences.
  • Assuming responsibility for establishing and maintaining effective working relationships with team members.
  • Identifying specific patient/caregivers needs and making referrals to appropriate team members.
  • Ensuring accurate, complete, succinct, and timely clinical documentation in accordance with HPCCR guidelines and the requirements of licensure and certification standards.
  • Demonstrating competency in knowledge and skills.
  • Ensuring that VIA professional reputation is maintained and projected.
  • Assuming responsibility for professional development and staying abreast of current trends in the healthcare field.
  • Incorporating new information and methods into practice.
  • Maintaining current professional licensure, CPR certification and CEU requirements.
  • Maintaining productive and professional relationships with external agencies and healthcare providers.
  • Exhibiting time management skills.
  • Planning work day while remaining able to respond appropriately to crisis.
  • Cleaning patient rooms as required per patient room cleaning procedure.
  • Ensuring all areas of facility are clean and appropriately stocked with supplies.
  • Keeping family room clean throughout day.
  • Ensuring proper chemicals are used for cleaning different surfaces.
  • Ensuring protective equipment is used when working with chemicals, i.e. gloves, glasses.
