This role involves providing compassionate, patient-centered personal care to hospice patients in their residences, including private homes and facility settings. Responsibilities include assisting with daily living activities, offering emotional support and companionship, providing bedside vigil support during end-of-life care, assisting with mobility and transfers, and safely using durable medical equipment. The CNA will also observe and report changes in patient condition, document care provided, foster a cooperative team environment, and provide support and education to family members and caregivers. Participation in required educational meetings and ongoing training is also expected.
