Requirements

  • Minimum 1 year experience required.
  • Current professional licensure.
  • CPR certification.

Responsibilities

  • Provides personal care (ADL) including bathing, body mechanics (turning and positioning, transfers, ambulation, range of motion), nutrition (feeding, intake/output, snacks), elimination (bedpan/urinal, specimen collection, catheter care, drainage devices), safety (side rails, restraints, CPR, infection control), and special procedures (vital signs, height/weight, heat/cold application, decubitus ulcer prevention/care, surgical preps, dressing changes, bandage application, postmortem care, cough/deep breathing assistance).
  • Performs housekeeping services necessary for patient/family/caregiver health and safety.
  • Participates in interdisciplinary team conferences (IDT) and special case conferences.
  • If functioning as a NA 1+ 4, provides personal care (ADL) plus sterile dressing for wounds over 48 hours old, wound irrigation, ostomy care or irrigation, replacement of existing urinary catheter (after nursing assessment), and oxygen therapy setup and monitoring.
  • If functioning as a NAII, provides personal care (ADL) plus fecal impaction removal, IV fluid-assisted activities (tubing setup/flush, flow rate monitoring, site care, discontinuation), nutritional activities (oral/nasogastric infusions, gastrostomy feedings, tube clamping/removal), suctioning (oropharyngeal, nasopharyngeal), tracheostomy care, replacement of existing urinary catheter (after nursing assessment), sterile dressing for wounds over 48 hours old, wound irrigation, ostomy care or irrigation, and oxygen therapy setup and monitoring.
  • Attends and participates in interdisciplinary team conferences [IDT] and special care conferences.
  • Assumes responsibility for establishing and maintaining effective working relationships with team members.
  • Identifies specific patient/caregiver needs and makes referrals to appropriate team members.
  • Ensures accurate, complete, succinct, and timely clinical documentation in accordance with HPCCR guidelines and licensure/certification standards.
  • Demonstrates competency in knowledge and skills.
  • Ensures VIA professional reputation is maintained and projected.
  • Assumes responsibility for professional development and staying abreast of current trends in the healthcare field.
  • Incorporates new information and methods into practice.
  • Maintains current professional licensure, CPR certification, and CEU requirements.
  • Maintains productive and professional relationships with external agencies and healthcare providers.
  • Exhibits time management skills.
  • Plans work day while remaining able to respond appropriately to crisis.
  • Cleans patient rooms as required per patient room cleaning procedure.
  • Ensures all areas of facility are clean and appropriately stocked with supplies.
  • Keeps family room clean throughout the day.
  • Ensures proper chemicals are used for cleaning different surfaces.
  • Ensures protective equipment is used when working with chemicals, i.e., gloves, glasses.
