About The Position
We have recently engaged with a very progressive, well run and financially stable Hospice organization in the Atlanta area of Georgia. They are a family-owned organization that is committed to patient satisfaction, clinical excellence, and quality care for each member of the family.
Requirements
- Registered Nurse in the State of Georgia
- 7 on 7 off – 7pm to 7am
- 2 years Patient Care Experience-Required
- Familiarity with EMR System
Nice To Haves
- 1 year of Hospice or Palliative Care Experience-Preferred
- KanTime-Preferred
Responsibilities
- Completes General Inpatient Admissions
- Working in a Hospital Setting
- Transitioning Patients to Home Hospice (Emory Hospital)
- Collaborate With The Physician and Mid-Levels to Determine Eligibility For Hospice Care
- Daily Patient Visits While in The Hospital
- Educates Patient About Home Hospice Care
- Implements Patients Care Plans
- Participates in Biweekly Meetings
