About The Position

Parker Jewish Institute Comprehensive Community Hospice is seeking a dedicated and compassionate full-time Nurse Practitioner (NP) to join their team. The ideal candidate will be empathetic and focused on providing high-quality end-of-life care. The Hospice Nurse Practitioner will provide nursing care to terminally ill hospice patients, offer assistance and understanding to families during home care and bereavement, and collaborate as a member of the Hospice team. The program aims to provide comfort, relief, and peace of mind to individuals with advanced or life-limiting illnesses and their families through personalized care that ensures comfort, enhances quality of life, and preserves dignity. The role involves conducting patient encounters to determine initial and continued eligibility for hospice services.

Requirements

Current New York State license as a Nurse Practitioner (NP)
Degree from an accredited NP program
1+ years of experience as an NP
Able to use computer technology and electronic medical records in the field
Access to reliable transportation and willing to conduct home visits

Previous experience in hospice or home care a plus

Responsibilities

Provide nursing care to the terminally ill Hospice patient.
Provide assistance and understanding to the family in the home care situation and in time of bereavement.
Work as a member of the Hospice team in providing Hospice care.
Conduct encounters with patients to determine their initial and continued eligibility for hospice services.
