About The Position

Parker Jewish Institute Comprehensive Community Hospice is seeking dedicated and compassionate full-time Nurse Practitioners to join their team. The ideal candidate will be naturally caring, empathetic, and focused on providing the best quality of end-of-life care possible for every patient. The Hospice Nurse Practitioner will provide nursing care to terminally ill Hospice patients, offer assistance and understanding to families in the home care situation and during bereavement, and collaborate as a member of the Hospice team. Community and Inpatient Hospice at Parker Jewish Institute provides comfort, relief, and peace of mind to individuals with advanced or life-limiting illnesses and their families. This intimate and unique not-for-profit program provides personalized care to ensure comfort, enhance quality of life, and preserve the dignity of patients. The role involves conducting patient encounters to determine initial and continued eligibility for hospice services. Parker Jewish Institute for Health Care and Rehabilitation, located on the Queens-Nassau County border in New Hyde Park, New York, is a non-profit health care facility offering inpatient programs (sub-acute/short-term rehabilitation, long-term care, nursing home care) and community health services (certified home health care, comprehensive community hospice program serving terminally ill patients in their homes or in nursing facilities). Requirements

  • Current New York State license as a Nurse Practitioner (NP)
  • Degree from an accredited NP program
  • 1+ years of experience as an NP
  • Able to use computer technology and electronic medical records in the field
  • Access to reliable transportation and willing to conduct home visits

Nice To Haves

  • Previous experience in hospice or home care a plus

Responsibilities

  • Provide nursing care to the terminally ill Hospice patient.
  • Provide assistance and understanding to the family in the home care situation and in time of bereavement.
  • Work as a member of the Hospice team in providing Hospice care.
  • Conduct encounters with patients to determine their initial and continued eligibility for hospice services.
