About The Position

As a Nurse Case Manager (RN Case Manager) you will be responsible for patient and family satisfaction through the provision of nursing care and case management by conducting assessments, creating and updating the plan of care, providing interventions, communicating, and documenting. The Nurse Case Manager provides patient and family centered care consistent with Agrace philosophy, goals, standards, regulations, and policies.

Requirements

  • ADN or BSN required
  • Current, unrestricted Registered Nursing License in state of employment
  • One-year nursing experience, preferred
  • Valid driver’s license, auto insurance, and access to a reliable vehicle
  • Must be at least 18 years of age
  • Basic Microsoft Office Skills
  • Intermediate electronic health records skills
  • Ability to use proper body mechanics, safe patient handling, and standard precautions

Responsibilities

  • Conducting patient and family assessments, including physical, pain, psychosocial, emotional, spiritual, environmental, and educational needs.
  • Collecting data from patients, families, significant others, interdisciplinary team members, and other healthcare professionals.
  • Identifying, interpreting, and documenting assessment data.
  • Documenting and reporting significant changes in condition or unusual incidents.
  • Collaborating with the Interdisciplinary Team (IDT) and primary care provider to determine hospice eligibility.
  • Responding to patient and family needs or changes in condition.
  • Providing interventions, treatments, and pain management.
  • Evaluating the effectiveness of interventions and education.
  • Formulating, implementing, and revising individualized Plans of Care (POC) in collaboration with the IDT, patient, and family.
  • Developing outcome measures and establishing care priorities.
  • Documenting interventions, outcomes, and changes in patient condition in the electronic health record (EHR).
  • Delegating visits to appropriate disciplines and managing visit sets.
  • Overseeing the work of Certified Nursing Assistants (CNA) and Licensed Practical Nurses (LPN).
  • Participating in meetings to plan and evaluate patient/family services.
  • Precepting new staff and students.
