About The Position

AdvisaCare is a home health and hospice provider focused on personalized in-home care and compassionate support for patients and families. The Hospice MSW Social Worker guides patients and families through the hospice journey by providing psychosocial, emotional, spiritual, and practical support. The role includes assessment, care planning, counseling, advocacy, referrals, documentation, and collaboration with the interdisciplinary team and community agencies.

Requirements

Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education (CSWE).
Active LMSW or LLMSW license in Michigan.
Prior experience working with patients and families.
Valid driver’s license, car insurance, and reliable transportation.
Eligible to work in the United States.

Responsibilities

Perform initial psychosocial, emotional, spiritual, and bereavement assessments and help develop and implement goal-directed care plans.
Provide care under a physician’s direction and participate in IDT conferences, observing, assessing, and sharing information about psychosocial, emotional, spiritual, physical, and financial conditions affecting patients and families.
Document patient and family visits, telephone contacts, and referral actions accurately, concisely, and in a timely manner in the patient record.
Reassess patient and family needs and provide counseling as required.
Provide crisis intervention and individual or family counseling when indicated.
Serve as a care advocate for the patient and family unit.
Develop and maintain contact with appropriate community agencies and services to promote interagency cooperation and facilitate referrals.
Provide ongoing counseling to patients and families about death and dying as needed.
Participate as a member of the Bereavement Team as assigned.
Adhere to all hospice policies.
