Alpha Home Health and Hospice is a Medicare-certified, 5-Star rated agency who is seeking an experienced and caring Full-time: Bachelor's Level Medical Social Worker (BSW) for our Hospice division in the greater Snohomish County area. Requirements

  • A graduate of an accredited bachelor's or master's program in social work
  • Minimum of one year’s experience in health care is required
  • Possesses and maintains current BLS Certification
  • A licensed driver with an insured automobile
  • Requires weekend rotation: Saturday and Sunday 8-5 or 9-6
  • A Tuesday through Saturday schedule is highly preferred but not required.

Nice To Haves

  • Experience with home health or hospice is strongly preferred

Responsibilities

  • Assess the psychosocial status of patients related to patient’s illness and progression
  • Develop and provide multiple community and governmental resources to support a patient in their home environment
  • Provides assessment in patient’s residence, carries out social evaluations, and plans interventions
  • Participate in the development of the individualized plan of care
  • Assist physician in understanding significant social and emotional factors
  • Participate in quality assessment performance improvement team
  • Serve as liaison between patients and community agencies
  • Maintain and develop contracts with public and private agencies
  • Assist with bereavement coordination
