Alpha Home Health and Hospice is a Medicare-certified, 5-Star rated agency who is seeking an experienced and caring Full-time: Bachelor's Level Medical Social Worker (BSW) for our Hospice division in the greater Snohomish County area. Requirements
- A graduate of an accredited bachelor's or master's program in social work
- Minimum of one year’s experience in health care is required
- Possesses and maintains current BLS Certification
- A licensed driver with an insured automobile
- Requires weekend rotation: Saturday and Sunday 8-5 or 9-6
- A Tuesday through Saturday schedule is highly preferred but not required.
Nice To Haves
- Experience with home health or hospice is strongly preferred
Responsibilities
- Assess the psychosocial status of patients related to patient’s illness and progression
- Develop and provide multiple community and governmental resources to support a patient in their home environment
- Provides assessment in patient’s residence, carries out social evaluations, and plans interventions
- Participate in the development of the individualized plan of care
- Assist physician in understanding significant social and emotional factors
- Participate in quality assessment performance improvement team
- Serve as liaison between patients and community agencies
- Maintain and develop contracts with public and private agencies
- Assist with bereavement coordination
