About The Position

We are seeking a dynamic individual to join our interdisciplinary Hospice team, where you'll have the unique opportunity to provide direct, one-on-one support to individuals in the comfort of their homes.

Requirements

  • Master’s degree from a school of social work accredited by the Council on Social Work Education
  • Minimum one year of progressive clinical experience in a medically-oriented setting.
  • Exceptional new graduates will be considered.
  • Valid driver’s license.

Responsibilities

  • Directly engage with patients in the comfort of their own home, providing one-on-one support.
  • Work collaboratively with a supportive interdisciplinary team, including medical doctors and caregivers, to develop personalized care plans.
  • Maintain consistent, timely, and accurate communication with all members of the patient’s health care team.
  • Regularly evaluate each individual's progress toward their care goals, updating the care plan as needed in collaboration with the healthcare team.
  • Participate in team meetings and collaborate with supervisors to identify challenges, and foster supportive and innovative solutions.
  • Deliver evidence-based interventions tailored to address psychosocial challenges impacting clients’ medical care.
