About The Position

Suncrest Hospice aims to redefine hospice care by offering exceptional service. This is achieved by increasing staff to patient ratios, leading to more clinical visits and lower clinician caseloads. Suncrest Hospice is a Community Health Accreditation Partner (CHAP) certified hospice. We are seeking individuals committed to providing the highest quality of care to patients and their families.

Requirements

• Current Licensed Vocational/Practical Nurse (LVN/LPN) licensure (in good standing) in the state(s) of practice.
• Great clinical and communication skills.
• Ability to work independently and manage daily activities, including phone, email, and written communication, meetings with healthcare professionals, and understanding of hospice rules and regulations.
• Time management skills to manage visits, documentation, and meetings, prioritizing and triaging multiple concerns.
• Ability to learn an electronic medical records system.
• Willingness to drive with reliable transportation, a Driver's License, and car insurance.
• Must live in Los Gatos, Glenwood, Aptos, Santa Cruz, Watsonville, Capitola, Scotts Valley, Felton, etc.

Nice To Haves

• Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Works closely with patients on a 1-1 level but gets strong support from a patient care team.

Responsibilities

• Complete patient visits as needed.
• Answer calls from patients, families, and customers.
• Conduct visits in patient homes, including private residences, assisted living communities, and skilled nursing facilities.
• Advocate for hospice services and Suncrest Hospice.
