About The Position

At Brighton/Suncrest Hospice, our goal is to change the expectation of hospice care by providing exceptional care and service to our patients. We achieve this by allocating resources to increase staff to patient ratios, thereby increasing clinical visits while lowering clinician caseloads. We are proud to be a Community Health Accreditation Partner (CHAP) certified hospice. If you have a commitment to providing the highest quality of care to patients and their families, we would like to hear from you! As a Weekend LVN/LPN, you will complete patient visits as needed. Visits occur in patient homes including private residences, assisted living communities and skilled nursing facilities. This LVN/LPN position must also be passionate about hospice care and be a great advocate for both hospice services and Suncrest Hospice.

Requirements

  • Current Licensed Vocational/Practical Nurse (LVN/LPN) licensure (in good standing) in the state(s) of practice
  • Great clinical and communication skills.
  • Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Work closely with patients on a 1-1 level but get strong support from a patient care team.
  • Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
  • Time management skills.
  • Ability to learn an electronic medical records system.
  • Willing to drive with reliable transportation, Driver's License, Car insurance.

Responsibilities

  • Complete patient visits as needed.
  • Answer calls from patients, families and customers.
  • Manage visits and documentation.
  • Attend required meetings.
  • Respond to urgent needs by prioritizing and triaging multiple concerns.
