As a Hospice LPN with Interim HealthCare of Oklahoma City, you'll deliver hands-on, one-on-one care and be a steady, compassionate presence when it matters most. Requirements

• Active LPN license in Oklahoma
• Hospice experience required
• Knowledge of hospice regulations and the Medicare hospice benefit
• Strong clinical skills, communication, and a heart for end-of-life care
• Reliable transportation and a valid driver's license

Responsibilities

• Provide hands-on hospice care in patients' homes, following the plan of care
• Manage symptoms and comfort, and support patients and families through end of life
• Document visits and report changes to the case manager and hospice team
• Work alongside RNs, aides, social workers, and chaplains
• Be a calm, reassuring presence for patients and the people who love them
