About The Position

Grace Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to provide end-of-life care to terminally ill patients. As part of an interdisciplinary team, the LPN will work under the supervision of the Patient Care Manager and in coordination with the RN Case Manager to ensure patients' loved ones, facility partners, and the hospice team are informed of any changes or needs. Grace Hospice focuses on providing professional care with a personal touch, emphasizing quality of life and dignity for patients and their families. Requirements

  • Graduate of an accredited vocational school of nursing
  • Current Minnesota LPN license
  • Strong interpersonal communication and collaboration abilities; both verbal and written to interact with patient, families and facility staff
  • Competency in electronic patient and hospice management systems
  • Current MN Driver’s License, insurance and daily access to reliable vehicle
  • Ability to critically think
  • Can build strong Interpersonal Relationships
  • Displays an attitude of courtesy and respect for all prospective patients, families, staff and referral sources
  • Functions as a team member with other staff in the performance of duties.
  • Seeks and accepts feedback regarding the performance of job duties.
  • Is aware of patient confidentiality and adheres to HIPAA regulations.
  • Displays an understanding of vulnerable adult issues.
  • Displays an understanding of hospice patients’ rights.
  • Exhibits sound mature judgment.

Nice To Haves

  • Previous nursing experience is preferred

Responsibilities

  • Provide direct patient care to terminally ill patients; identify and address their individual physical, psychosocial, safety, and comfort needs as directed by the care plan and RN case manager
  • Manage chronic and acute end-of-life symptoms and medications
  • Provide wound care, personal hygiene, and assist with feeding as needed
  • Train and educate patients, families, caregivers, and partner facilities on end-of-life care
  • Collaborate with the interdisciplinary team and others involved in the patient’s care
  • Participate in bi-weekly IDG meetings
